Handouts

Handouts can be used for educational purposes to explain to patients, parents, or other rehabilitation professionals why, how and what interventions you are implementing with patients.

 

Types of Goals—Understanding the three main types of goals is great for guiding goal-setting and assuring that the individual sets the right goals for success.

This information should be used when creating goals for athletes so they know what kinds of goals are most appropriate and what they mean.
 

SMART goals—SMART goals gives an outline of what a good and positive goal looks like and how to guide goal setting.

Thinking about goals—Goal setting is important to know what you are trying to achieve, but sometimes over thinking about long term goals can overwhelm you or cause anxiety, so understanding when to bring your attention to certain goals can promote achievement.

This handout could be used at the time of goal setting, or if an athlete is seemingly overwhelmed by the goals they have set and their achievement of them.
 

 
 

Worksheets

Worksheets are interactive for the patient to complete on their own time or with the help of the rehabilitation professional.

 

Goal-mapping—This goal map organizes outcome, performance, and process goals in a concise way. Writing down goals increases efficacy and accountability.

Goal setting should be done several times through rehab, as they move through the phases of injury. Revisiting this goal map and creating new ones is an ongoing journey.

This Week in Rehab—Using this to determine what is important, why it is important, and preparing ways to accept and re-evalute if the goals aren’t achieved. It’s also important to recognize that every day, rehab session, week, or month not go as planned—that’s being human. But it is important to prepare for those bumps in the road and be easy on yourself in the process.

Goals-What, why, how—Creating goals is important, but creating a plan and understanding why a goal is a goal is also important.

Once someone has created their goals, understanding why the goal is important, and how they are going to achieve it is the next step. This should also be revisited as goals are completed and re-set.

Goals to be proud of— Making sure the goals you make are goals that you can be proud of can improve achievement and enjoyment of pursuing the goals.

As with the other goal setting worksheets, this should be revisited with the completion of goals. However this one is unique in that it is especially important if an athlete is struggling to determine meaningful goals for themself.
